Klarus Home Health / Jet Health, Inc is seeking a full time, part time or PRN (per diem) dedicated and professional Weekend Admissions Physical Therapist to join our team serving our patients enrolling in our home health services in Denver Metro area of Colorado and surrounding areas.
This role involves providing coordinated care to patients/clients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being. The Physical Therapist will plan, organize, and conduct physical therapy treatment programs based on medical referrals and their evaluations, ensuring effective coordination of care.
