Weekly Hour Guarantee: 40 hours Productivity Expectations: 30 points Coverage Areas: Morris County & Essex County, NJ Shift Schedule: Monday–Friday, 8:00 AM–4:30 PM On‑Call / Weekend Requirement: One weekend per month, if possible Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient Pay Rate: $49/hr
Top Responsibilities
Evaluate and treat patients in their homes according to individualized care plans
Develop PT treatment plans based on clinical assessments and patient goals
Document patient progress accurately and in compliance with agency standards
Coordinate with interdisciplinary teams to ensure comprehensive patient care
Educate patients and caregivers on exercises, safety, and functional mobility
