Massachusetts RN License with Primary Source Verification required within 30 days of start date.
Fit Test for N95 respirator within 365 days of start date, updated annually.
Respirator Medical Clearance Attestation required if fit test is required.
MGB Medication Exam with 80% passing within 365 days.
MMR vaccination or proof of immunity required.
TB test documentation within 3 months of start date, updated if >6 month break.
Health Statement signed by MD/DO/NP/PA within 365 days of start date.
MGB Criminal Background Check within 45 days prior to start date.
MGB Massachusetts CORI background check within 30 days prior to start date.
MGB Resume including past 7 years of work experience.
2 references within 4 years, minimum 1 supervisor or above in skillset.
