Our client is seeking an Adult Social Worker to join their Young People Transitions to Adulthood Team based in Oldham.
What will your responsibilities be?
Support young people preparing for adulthood, completing holistic assessments that identify aspirations, needs, risks, and the support required as they move from children’s to adult services.Coordinate multi‑agency transition plans, working closely with education, health, adult social care, and community providers to ensure continuity of care and smooth handover.Promote independence and person‑centred outcomes, developing plans that build life skills, enhance wellbeing, and empower young people to make informed decisions about their future.
