Carry out assessments and develop care plans for individuals within the community, ensuring their needs are met in line with statutory responsibilities.Work collaboratively with health professionals, housing teams, and voluntary services to coordinate support and promote independence.Respond to safeguarding concerns, manage risk effectively, and ensure timely interventions to protect vulnerable adults.
