Functions as a Senior Medical Social Worker to provide bio‑psychosocial assessment and intervention in the management of patient / resident / client care in line with professional standards, while supervising a team and leading projects.
Provide end‑to‑end counselling and case management for complex / specialised cases, including assessment, care planning, intervention, and evaluation.
Lead the development, facilitation, and evaluation of group work sessions to strengthen social support systems and networks for target groups.
Identify service gaps and advocate for or lead the development of programmes to improve service delivery for patients / residents / clients / caregivers and stakeholders.
Plan, manage, and deliver programmes for clients, families, multidisciplinary teams, and relevant stakeholders in specialised areas of health social work.
