Build trust-based relationships with residents and local stakeholders in Tengah New Town and surrounding precincts to identify community strengths, informal support networks, and shared priorities.
Conduct placed-based outreach, including 1:1 conversation, to understand individual and community aspiration, using both lived experience and data to inform engagement strategies.
Facilitate community-led efforts that promote social connection, mutual support and neighbourhood level wellbeing.
Support residents in navigating health and social systems, with an emphasis on enabling independence and long-term resilience.
