The Director of Case Management oversees all departmental activities, ensuring compliance with organizational policies and regulatory agencies while leading the integration of patient care and discharge planning processes with hospital departments and external agencies for continuity of care and optimal Resource Utilization. The Director manages daily operations, engages interdisciplinary teams and stakeholders, and is accountable for achieving clinical, service, and financial outcomes.
