Reporting to the Care Manager, the Case Manager is responsible for directing the care of clients under the home care program.
Responsibilities
- Identify client and family needs using assessment and organizational skills.
- Facilitate appropriate internal and external resources with the interdisciplinary team.
- Coordinate care planning and facilitate client‑family teaching.
- Develop and modify care plans on an ongoing basis as needed.
- Ensure timely referrals to stakeholders and provide direct care as required.
- Deliver wound care, palliative support, and home parenteral therapy.
- Lead and support an integrated, holistic approach to patient care, health promotion and maintenance.
Compensation
