Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours. Completes re-visit documentation and transfers.
Uses age-appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary. Documents patient education tools used.
