**Job Description**

**UW Medical Center - Montlake's Social Work and Care Coordination department** has an outstanding opportunity for a **Cardiac** **Nurse Care Coordinator** to join our team, with the goal of achieving coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge. This position works closely with all members of the interdisciplinary health care team. ensuring that the team is aware of the patient’s plan of care and that all disciplines are working toward the same goals for the patient.

**WORK SCHEDULE**

+ 50% FTE, Days
+ Wednesdays to Fridays

**PRIMARY JOB RESPONSIBILITIES**

+ Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures and tests, ongoing team communication, and facilitates coordination between providers across the continuum of care.
