Position Summary
The Regional Cardiac Care Coordinator (RCCC) is an integral member of the Cardiac Care team, responsible for supporting timely access to advanced cardiac services and ensuring high‑quality coordination of care across the patient journey. Working collaboratively with cardiologists, cardiac surgeons, diagnostic departments, and Ontario Health–CorHealth, the RCCC facilitates referrals, monitors patient progress, supports transitions of care, and provides education to patients and families.
This role upholds the principles of Patients First by improving access, informing patients, connecting services, and supporting the sustainability of Ontario’s cardiac care system.
