London – Senior Adult Social Worker – £30 to £32

• Manage a complex caseload of adults with diverse care and support needs.
• Undertake Care Act assessments, reviews and support planning.
• Complete Mental Capacity Assessments and Best Interest decisions where appropriate.
• Lead safeguarding enquiries in line with the Care Act 2014.
• Work collaboratively with health professionals and multidisciplinary teams.
• Develop person-centred support plans that promote independence and wellbeing.
• Coordinate complex discharge planning and community support where required.
• Provide professional guidance and support to less experienced colleagues.
• Prepare high-quality reports and maintain accurate case records.
• Ensure statutory responsibilities are delivered in accordance with legislation and local authority policies.
