An exciting opportunity has arisen for a Practice Pharmacist to join our dynamic and forward-thinking team at Brookside Group Practice. We are looking for 15-20 hours per week.

Job responsibilities

• Provide medication review and query services to patients via telephone or face-to-face clinics as appropriate.
• Ensure data is recorded so patient care is enhanced and can be retrieved for data analysis.
• Attend and contribute to various practice meetings as requested.
• See patients with single or multiple medical problems, including long-term conditions where medicine optimisation is required.
• Provide pharmacist support to clinicians attending care homes or to attend themselves as appropriate. Work with care home staff to improve safety of medicine ordering and administration.
• Manage caseload of patients with common/minor/self-limiting ailments within the scope of practice and limits of competence. Signpost to comm...
