Job Summary:
The WelbeHealth Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function.
The Physical Therapist collaborates with the Occupational Therapist to efficiently operate WelbeHealth rehabilitation department.
Physical Therapist is a member of the WelbeHealth management team and facilitates collaborative relationships across WelbeHealth and with external stakeholders to promote the WelbeHealth values, team culture and mission.
