Overview


The Pharmacist performs pharmaceutical care activities for customers/patients within the service area; monitors, documents and reports patient outcomes; provides or coordinates the dispensing of medications, home medical equipment and ancillary supplies within the service area; and participates in on-call schedule.


Responsibilities


+ Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals

+ Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice

+ Adheres to all appropriate standards of accreditation and participates in quality assurance activities
