TITLE: Clinical Pharmacist Inpatient

JOB OVERVIEW: The Inpatient Clinical Pharmacist is an integral member of the patient care team providing pharmacotherapeutic recommendations to providers, nursing, and other members of the health care team to optimize patient care. Responsibilities include, but are not limited to direct patient care, medication therapy management, drug therapy consultation via collaborative drug therapy agreements (CDTAs) drug dispensing, participation in transitions of care, and provision of patient education and counseling for assigned patient care areas.
