A pharmacy role focused on Transitions of Care and Ambulatory Care is available at a large 400-bed hospital in the San Fernando Valley region of Los Angeles. This position offers an opportunity to impact patient care by working closely with individuals prior to discharge, ensuring they understand their medications and have necessary refills in place to support adherence and positive health outcomes.

Responsibilities:

• Provide medication education and counseling to patients before discharge
