Serves as a drug information expert and resource regarding medication management.
Provides comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services.
Participates in antimicrobial stewardship and opioid stewardship.
Provides medication counseling to patients.
Works with inter-professional patient care teams to help patients with transitions to care, including=but not limited to helping prepare patients for discharge.
Serves as an active member of the inter-professional team on patient care units. Participates in unitbased huddles and activities if available. Knowledgeable in unit’s metrics, goals, and works with staff on unit to help achieve goals.
