Summary This position serves as a Clinical Pharmacy Specialist (CPS) in the area of Anticoagulation within the Pharmacy Service at the VA Sierra Pacific Network (VISN 21). The CPS functions at the highest level of clinical practice and works under an approved scope of practice to provide comprehensive medication management, evidence-based anticoagulation therapy oversight, and advanced clinical decision support.
