Our client is looking to recruit a Minor Injuries/Illness ACP to join their team of clinicians and deliver the best care to patients in the area. The role involves managing episodes of care for patients with minor injuries and managing acute undifferentiated presentations across Urgent Care Services. The Minor Injuries ACP will work as an autonomous practitioner, assessing, diagnosing, and planning care, including prescribing where qualified.

Responsibilities

• Manage acute presentations to the Walk‑In Centre as part of a team of autonomous practitioners.
• Determine urgency and best pathway for patients presenting to the Emergency Departments (streaming).
• Utilise clinical knowledge to manage patients presenting with acute, undifferentiated problems and those with chronic long term conditions.
