Exciting opportunity to join Endeavor Health in the Department of Medical Genetics – High Risk Breast Services. Functions in the role of expert clinician, educator, consultant and coordinator of care. In collaboration with the Physicians and Genetic Counselor's, the APN plans, coordinates, monitors and evaluates the clinical care and services for patients and families by providing clinical support to improve access to care, patient care, and outcomes. The APN serves as a resource and primary contact to assist the patient and family in navigating healthcare and coordination of care/treatment in Personalized Medicine.
