We are seeking a motivated and compassionate Band 6 Mental Health Practitioner to join our dynamic Locality Mental Health Team, providing high‑quality, recovery‑focused care to adults with complex and enduring mental health needs. You will work both autonomously and as part of a supportive multidisciplinary team, delivering evidence‑based interventions that promote safety, wellbeing and independence.
As a key member of the team, you will hold a defined caseload, act as a lead HCP and deputise for the team manager when required. You will play an active role in assessment, formulation, risk management and collaborative care planning, ensuring that service users and carers are at the centre of decision‑making.
