Responsibilities - Provide psychosocial support to patients with special needs and their families through casework, counselling, and therapeutic group interventions.
- Collaborate with a multidisciplinary healthcare team to deliver holistic and integrated care for patients.
- Actively participate in case discussions, care planning, and family conferences.
- Engage in practice research and quality improvement projects to enhance clinical social work standards.
Requirements - A degree in Social Work or a Graduate Diploma in Social Work.
- A post-graduate qualification in Counselling or Family Therapy is advantageous.
- Competence in IT applications and documentation systems.
- Strong interpersonal and communication skills, with the ability to thrive in a multidisciplinary team.
