**Position Purpose**

Trinity Health at Home is seeking a compassionate and skilled **Medical Social Worker (PRN)** to provide consultation, assessment, and support services to homebound patients and their families. In this role, you will help address complex social and emotional challenges that may impact a patient’s ability to manage their medical condition, promoting improved health outcomes, independence, and quality of life.

**What You Will Do**

+ Conduct comprehensive psychosocial assessments of patients and caregivers.
