**BAYADA Home Health Care** is currently seeking an energetic and experienced **Medical Social Worker, MSW,** for a **full time** opportunity performing **hospice visits** **our Norwich office.** This office services clients on a per visit basis in territories throughout Windsor and Orange, VT and Grafton NH counties.

**Travel from either:**

**St. Johnsbury to Norwich area (approximately)**

**OR**

**Norwich to Ludlow, VT area (approximately)**

Prior home care experience preferred, but not required. As a Medical Social Worker, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and palliative care to clients, affording them the opportunity to receive the care and support required to remain at home.
