Job Summary:
We are seeking an experienced and detail-oriented MDS Coordinator to join our team and support three of our facilities (2 in Frostburg and 1 in Cumberland). As an MDS Coordinator, you will play a crucial role in ensuring the accurate and timely completion of Minimum Data Set assessments for three of our facilities. You will review medical records, collaborate with the care team, communicate with physicians and families, and maintain a comprehensive plan of care for each patient.
