Position Summary
The site nurse is responsible for the ongoing, overall healthcare service delivery, coordination and monitoring of all assigned individuals.
Essential Functions
Completes regular site visits to all assigned sites to make formal and informal observations of staff performance, individual health needs, ensures implementation of changes in plans of care.
Responsible for review and documenting the ongoing health needs of assigned individuals and works with all members of the individual’s team to formulate, update and document associated plans of care, nursing goals and objectives and interventions.
