Duties Description Are you ready to start your Social Work career with New York State? Buffalo Psychiatric Center is recruiting for a Licensed Master Social Worker to provide professional social work services to individuals with mental illness. Candidates from outside or within State Service can apply.

As a Licensed Master Social Worker, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness.

Duties include:

• Providing crisis intervention, service linkages, and discharge planning
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
• Determining discharge needs and appropriate community resources
• Establishing and maintaining relationships with community providers to ensure continuity of care.
