Licensed Clinical Social Worker - Integrated Behavioral Health-Primary Care-$7,500 SIGN ON BONUS

Position Summary
The LCSW provides integrated behavioral health services within the primary care setting, delivering assessments, brief therapeutic interventions, and care coordination for adolescent and adult patients. This role supports improved psychosocial functioning, facilitates access to appropriate levels of care, and collaborates closely with providers and the multidisciplinary team to ensure comprehensive, patient centered care.

Key Responsibilities

Conduct behavioral health assessments and develop individualized treatment and transition plans.

Provide evidence-based interventions (CBT, DBT, MI, SBIRT, mindfulness, trauma informed care).

Manage acute behavioral health concerns, including crisis intervention and suicide risk evaluation.
