CHC is looking for a dedicated registered nurse to join our family practice team at our brand-new muscatine clinic! In this position, you will learn a variety of skills while providing comprehensive, patient-centered care to individuals and families in our community.
salary: $31–$40/hour
in this key role, you will:
- health coach – develop care plans and partner with patients to achieve their health goals
- patient advocate – serve as a liaison between patients, families, providers, hospitals, and community agencies
- connector – identify and link patients to resources that support access to care
- social determinants of health – recognize and address psychosocial, cultural, ethnic, and spiritual needs of patients and families
