Intake LPN- Providence Home Services Oregon- Portland, Oregon (Halsey Building)



**Schedule** : Full Time, Monday-Friday on Day shift



Under the direction of the Administrator of Providence Home Care this role facilitates referrals, coordinates staff and assists patients and families with transitions into home health care. The Intake Specialist communicates with physician, hospital staff, patients, caregivers, and agency staff in order to provide continuity of care.
