We are seeking dedicated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our team in a per diem, temp-to-permanent role . This position involves conducting in-person, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members. Assessments are performed in members' personal living spaces in accordance with health plan expectations.
Schedule: Create your own Schedule, Hours of Operation are 7am-7pm Mon-Sat Visits: 15 per month minimum Travel: Required within a 50-mile radius from your residence Mileage reimbursement after the first 30 miles driven per day (IRS rate) Essential Functions - Complete required self-paced and instructor-led training within allotted time
