Provide personal care and supportive services to individuals in the community, in the privacy of their home.
Essential Functions
Provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.Immediately report changes in client’s condition to supervising professional.Ensure the safety of the client, client’s family and/or the employee.Maintain HIPAA.
