The compensation for this role is $28.21/hr for a 37.5 hour work-week.  DESCRIPTION:

• Work collaboratively as part of an interdisciplinary team that provides in-home services to older adults and individuals with disabilities.


• Make home visits, conduct comprehensive assessments, and participate in the development of person-centered care plans.

QUALIFICATIONS:

• Bachelor's Degree or higher.
