We are seeking a PRN Physician (MD/DO) to provide coverage in skilled nursing and long-term care facilities in the area of Murfreesboro, Cookeville, McMinnville, and Columbia.

Responsibilities

• Provide primary and transitional care services to patients in skilled nursing and long-term care facilities
• Perform patient assessments, diagnoses, and treatment planning
• Manage acute and chronic medical conditions
• Complete required documentation and Medicare recertification visits
• Collaborate with facility staff and interdisciplinary care teams
• Educate patients and families regarding treatment plans and goals of care
