We are seeking a PRN Physician (MD/DO) to provide coverage in three skilled nursing and long-term care facilities in the Chattanooga and North Georgia region .
Responsibilities
- Provide primary and transitional care services to patients in skilled nursing and long-term care facilities
- Perform patient assessments, diagnoses, and treatment planning
- Manage acute and chronic medical conditions
- Complete required documentation and Medicare recertification visits
- Collaborate with facility staff and interdisciplinary care teams
- Educate patients and families regarding treatment plans and goals of care
