**Position Summary**

The Family/Housing Specialist will provide support and service linkages to tenants who reside within the residence. They will conduct intake assessments, develop service plans, coordinate services, record case notes according to the New York City Department of Health and Mental Hygiene standards and ensure quality of care. The support plan will focus on the strengths and needs of the target population as well as the children living in the household.
