• Case Management : Independently manage routine cases assigned by MSWs, ensuring patients receive appropriate care and support.
• Financial Assessments : Conduct assessments to determine patients' eligibility for various financial assistance schemes, ensuring timely and accurate processing.
• Field Visits & Discharge Coordination : Perform home visits and accompany patients to social service organizations as needed. Support the discharge process by facilitating a seamless transition for patients back into the community or home care.
