Community Mental Health Practitioner

Join our team of dedicated professionals providing specialist mental health care to residents in Stockport. The role is part of the Secondary Care Community Mental Health Team (CMHT) supporting individuals with severe and enduring mental health conditions. You will work with people aged 16 and over, delivering holistic assessment, interventions, and risk management according to the team’s recovery and social inclusion framework.

Main duties

• Act as the community Mental Health Practitioner and key worker responsible for a defined group of clients.
• Provide comprehensive assessment, interventions, risk assessments and develop care plans for the caseload.
• Deliver depot medication administration for clients who cannot attend the depot clinic (where applicable).
• Provide supervision to allocated staff and students on placement within the team.
