Job Summary

 The Clinical Pharmacist is responsible for the provision of clinical pharmacy services including: anticoagulation, pharmacokinetics, antibiotic stewardship program, drug dosing consultation, nutritional support monitoring, drug information, formulary reviews, in-services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and pharmacy staff.
 This includes the selection of medications to disease state, monitoring of patient’s therapeutic outcomes, follow-up on medication errors and evaluation of adverse drug reactions.
 Provides prescribed medications, other pharmaceuticals and compounded medications according to professional standards and practices, hospital policies and procedures, regulatory agencies, and Federal and State laws....
