Responsibilities

• Lead and support PSWs and HSWs to ensure practices comply with all legislated standards of practice and company internal protocols.
• Respond to and mentor staff, patients/families, and other team members with regular touchpoints about concerns from the community.
• Provide ongoing leadership through regular huddles, face-to-face meetings and one-on-ones.
• Initiate patient safety incident investigations and document findings to ensure timelines and action plans are met.
• Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
