The Case Manager advocates for, and assists clients with, gaining access to needed medical, social, educational, or other services to address identified needs and achieve specific goals. The Case Manager provides ongoing assessment and support to engage potential clients and promote links to services during a critical engagement point. The Case Manager provides referrals and information to increase education for clients and families on available resources, increase support networks, and coordinate care among multiple service providers such as Drug Court, Mental Health/Substance Abuse Treatment Team, Department of Community Based Services, Juvenile Justice, schools and health care providers.
