We’re hiring a Case Manager to support individuals and families—especially those with SPMI—in achieving stability and independence.

What you’ll do:

• Assess clients & develop service plans

• Connect to housing, healthcare & community resources

• Provide ongoing support, advocacy & documentation

What we’re looking for:

Bachelor’s 

Experience with SPMI or vulnerable populations

Strong communication & organizational skills
