By 2030, almost a million Singaporeans will be aged 65 and above, and there will be an increased demand for aged care services (e.g., Senior Care Centres, Active Ageing Centres, Home Personal Care, Nursing Homes). However, the care landscape is fragmented today and care journeys for seniors can be uncoordinated with multiple providers delivering multiple assessments and care plans. The Sector Strategy and Development (SSD) team under the Ageing Planning Office is at the forefront of re-imagining the senior’s care journey, ensuring that seniors can receive seamless and coordinated care, and that service providers are uplifted to play such a role.
We will be introducing a new service model called the Integrated Community Care Provider (ICCP), where ICCPs would assess and plan for the senior’s care holistically, and be the designated point of contact to pull in and coordinate the aged care services that the senior needs.
