INFORMATION AND TRIAGE - Manage case referrals.
- Conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.
- Refer clients to appropriate agencies for further intervention.
- Ensure proper documentation of referrals and intakes.
- Ensure reports are submitted in a timely manner.
COMMUNITY WORK & GROUP WORK - Assist in implementation of community outreach / initiatives.
- Assist in implementation of groupwork sessions.
- Assist in administrative needs.
ADMIN - Assist in facility management related functions.
- Assist in data management and reporting.
- Assist in other administrative functions tasked by Centre Management Team (CMT).
