Ambulatory Pharmacist - Anticoagulation Clinic

The successful candidate will practice under a Therapy Management Contract to manage anticoagulation therapy through in person and virtual patient interactions. Responsibilities include targeted and comprehensive medication review, patient counseling and education, disease state and medication therapy management, drug information, and addressing barriers to medication access and adherence. Additional responsibilities include tracking and reporting productivity and quality metrics, involvement in the system-wide AC Clinic Collaborative and design and conduct of quality improvement activities. The schedule for this position includes a rotating weekend staffing commitment in the inpatient setting. Opportunities exist to serve as a mentor and educator for pharmacy residents, pharmacy students, pharmacists and healthcare professionals across other disciplines.
