– Degree in Social Work
– SWE Registration
– Right to work within the UK
Experienced social worker required to work in fast paced MDT team. Social Worker require to commission short term care, Complete Care Act Assessments, consider commission of long term care and complete referrals such as community alarm and invstigate any safeguarding concerns related to people under the North H@H MDT Service.
