Our HNF Wellness Day Care & Rehabilitation Centres offer an array of social activities, social support and holistic healthcare services to help our elderly stay independent in their own community.

• Lead and support ACP discussions, helping clients explore and completing advance care meet clients’ needs.
• Document and update client ACP preferences for submission accurately, ensuring proper record-keeping with proficiency in administrative processes.
• ACP Facilitators can either source their own leads or receive leads.
• Provide feedback on ACP services for quality improvement.
