Nurse Practitioners in the Personal Support and Stabilization (PSS) team are the medical leaders that provide medical care through a consultative lens to house bound frail patients recently discharged from hospital. The NP works within a dynamic interdisciplinary team of re-ablement providers: Physiotherapy, Occupational Therapy, Rehabilitation Assistants, Registered Nurses and Licensed Practical Nurses.
The ideal candidate must be able to work a flexible work schedule, possibly including weekends. This is a well-established role within a matrix of PSS NPs and within the Geriatric Community of Practice.
